The 9/11 Health Insurance Group and Group Health Incorporated (GHI) are pleased to offer a health benefits program to widows, widowers and their dependent children who lost health insurance coverage as a direct results of the events of 9/11. This program provides a broad range of medical, hospital, and prescription drug benefits.

• Rates Lower Than Individual Coverage Options. The GHI rates are generally lower than most plans available to individual customers.
  
  
• Vast Network of Participating Providers. GHI's Tri-State Network includes 130,000 participating provider locations, including virtually every general acute care hospital in New York State.
  
  
• No Physician Gatekeeper. With the 9/11 Health Insurance Group's PPO Benefits Program, you can see any GHI participating provider, including specialists, anytime you need care, for just a small copayment. No physician referrals are required.
  
  
• Out-of-Network Coverage. Under this PPO program, you can also receive most covered services from out-of-network providers, subject to deductible and coinsurance. You are also covered for emergency services in any hospital nationwide.
